FAMILY RELEASE, WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

(The "Agreement")

By checking the box above, you ("Primary Participant"), the secondary person ("Secondary Participant"), and any children listed ("Children") agree to the terms of this Release, Waiver of Liability, Assumption of Risk, and Indemnity Agreement ("Agreement") with The Final Percent LLC ("Organization"). This Agreement governs participation in any event, activity, or program provided by The Final Percent LLC.

1. ACKNOWLEDGMENT OF RISK

By participating in the activities offered by The Final Percent LLC, you, the Secondary Participant, and any Children acknowledge and accept that the activities involve inherent risks, including but not limited to:

Personal injury (e.g., falls, collisions)
Emotional distress
Property damage
Health risks such as exhaustion or dehydration
Risks associated with online programs, including technical difficulties

You voluntarily assume all risks for yourself, the Secondary Participant, and any Children participating in the activities.

2. WAIVER OF LIABILITY

You, the Secondary Participant, and the Children release and discharge The Final Percent LLC, its officers, agents, employees, and affiliates from any and all claims, damages, or liabilities arising from participation in activities, including those caused by negligence.

This waiver includes but is not limited to:

Personal injury, death, or property damage
Claims based on negligence, breach of contract, or any other legal theory.

3. INDEMNIFICATION

You agree to indemnify and hold harmless The Final Percent LLC from any claims, damages, or liabilities resulting from the participation of yourself, the Secondary Participant, or the Children in any activity, including claims brought by third parties.

4. CONSENT TO MEDICAL TREATMENT

In the event of an emergency, you authorize The Final Percent LLC to obtain medical treatment for yourself, the Secondary Participant, or the Children. You agree to bear the full cost of such treatment and release The Final Percent LLC from any liability related to medical care.

5. PHOTO, VIDEO, AND MEDIA RELEASE

You grant The Final Percent LLC permission to use photos, videos, or media of you, the Secondary Participant, and the Children taken during activities for promotional purposes without compensation.

6. VOLUNTARY PARTICIPATION

You confirm that participation is voluntary, and you are not aware of any health conditions that would make participation unsafe for you, the Secondary Participant, or the Children.

7. FAMILY AGREEMENT

By checking the box below, you agree that this Agreement applies to you, the Secondary Participant, and any Children listed below. You affirm that you have the authority to agree on behalf of the Secondary Participant and the Children.

8. SEVERABILITY

If any part of this Agreement is found invalid or unenforceable, the remaining terms will continue in full effect.

9. GOVERNING LAW

This Agreement is governed by the laws of Colorado The United States Of America.
